An exciting opportunity has arisen for a Band 6 Occupational Therapist to work within Eden ICC (Integrated Community Care) , in the beautiful Eastern Lake District, Cumbria .We have a full time Community Band 6 Occupational Therapy position available which involves weekend working, and our service covers the hours from 8-8pm. Flexible working patterns can be considered.

The ICC is a fast paced service with our main aim to prevent hospital admissions and facilitate discharges. As a Band 6 in the ICC you will need to work flexibly to meet the needs of the service. The role is varied, as you will be leading on D2A's (Discharge to Assess), POD (triage role) alongside managing your own caseload of patient's.

The post holder willmanage a defined caseload of patients, aged over 18 with a wide range of health and occupational challenges. Using evidence based/client centred principles to assess, plan, implement and evaluate interventions in a community setting. They will provide leadership and supervision to support staff and students within the team.

The post is a community position working across a rural area, however there is an expectation that you will work on the in-patient unit when service needs arise. The post holder will be required to be able to travel independently to locations where patients reside.

Main duties of the job

The Community Care Group provides a range of adult community health services to people living within North Cumbria. It's key community health focus is to provide care to prevent hospital admissions, facilitate early discharge and where possible to enable patients to manage their own conditions and remain in their own homes, enhancing both their independence and quality of life.

The Community Therapy Team in the Eden ICC provide specialist assessment and rehabilitation for patients with a wide range of long and short-term conditions, working closely with our health and social care colleagues.

Patients are seen as in-patients (in Community Hospital beds or Interim Community beds); and in their own homes (including residential and nursing homes).

There is a strong focus on professional development opportunities and CPD within the team. You will have access to high quality, regular clinical supervision, and we offer an induction, ongoing professional support and a highly rewarding job to the successful applicant.

The core working hours of the service are Mon to Fri 8.00-16.00 but the post holder will be included on a rota to cover D2A shifts, late shifts, weekend working and bank holidays. This is to support ICC working 7 days a week, 365 days a year 8am until 8pm.
