Job summary

We are looking for a Band 3 Health Care Assistant ideally with community nursing or caring experience to join our community nursing team, working between the hours of 8.30 am and 22.00 pm. This service is provided 7 days per week, and you will work alongside a team of experienced Nurses and Paramedics delivering care to the people of Swindon.

In this busy environment, you must be keen to learn and adapt to a changing environment as well as being expected to complete the Care Certificate training if you have not already achieved this and ideally have an NVQ level 3. You will need to be IT literate as there is an expectation to be competent in the use of an electronic patient record system following full training.

To fulfill the needs of service delivery to patients/service users and in line with our current team of clinicians, the post holder will work as part of a flexible rota over seven days a week, which will include working evenings, weekends and Bank Holidays.

A clean driving license and access to a vehicle is essential for this role.

This service will be transitioning to HCRG Care Group from the 1st April 2025. The successful candidate may transfer over (under Transfer of Undertakings (Protection of Employment) Regulations 2006) on the terms and conditions they are employed on. Main duties of the job

  • To work within a multi-professional team.
  • To provide treatment as delegated by a registered practitioner and following an appropriate by care plan.
  • To keep the patient at the centre of his or her care, ensuring patient and carer participation in decisionmaking.
  • To undertake the daily care and support of delegated patients.
  • Deliver treatment/care within a community setting as specified in a written care plan, under the guidance of a registered practitioner.
  • To practice the principles and philosophy of individualised patient care in accordance with policy, procedures and regulatory body's guidelines.
