An exciting opportunity has arisen for a Specialist Nurse Practitioner to join the Older People's Mental Health Services in the Mild Cognitive Impairment clinic.
The Mild Cognitive Impairment Clinic will operate across three Boroughs: Ealing, Hammersmith and Fulham and Hounslow.
This will be a small dynamic team and will need an enthusiastic professional to provide a leading role in offering patients assessment of their cognition. The post holder, working under the line management and supervision of the CIDS service manager, will be expected to undertake in depth assessment of patients with a who are referred to the MCI clinic, to liaise with consultant psychiatrist and formulate diagnoses for patients, in accordance with the objectives and vision of the Trust.
Main duties of the job
The post holder will be expected to undertake in depth assessment of patients who are referred to the MCI clinic. They will liaise with the consultant psychiatrist and formulate diagnoses for patients in accordance with the objectives and vision of the Trust.
The post holder will support in identifying the cohort of patients for assessment within the MCI clinic and ensure that they are offered follow up and review at an appropriate time. The post holder will be expected to manage a caseload and work independently as well as part of the wider clinic team. The post holder will have a leadership role and will ensure high standards of care to patients using the service.

- Educated to Masters level or equivalent experience
- Evidence of continuous professional development.
Desirable
- QI trained (IA/coach/QSIR)
- Master degree (or working towards)
- Recognised Teaching Qualification
- Non medical prescribing
Experience

- Demonstrable experience of conducting assessment for patient with cognitive impairment and dementia
- Experience in mental health nursing across a range settings i.e. inpatient and community
- Substantial recent experience in practice development / service improvement
- Ability to work under pressure and to tight and often changing deadlines
Desirable
- Previous experience in working within a clinical governance framework
- Experience in QI projects
- Proven ability to work at a strategic and senior leadership level

- Knowledge of national policy relating to peoples mental health including dementia
- Must have an understanding of the background to and aims of current healthcare policy and appreciate the implications of this on engagement
