Discharge Case Manager

Job summary

Are you ready for a challenge? The Discharge Team is looking to appoint Discharge Case Managers. The role of the Case Manager will be to support the Discharge to Assess process while facilitating safe discharges from the hospital and supporting flow and capacity within the Trust. This role will cover a seven-day service, including bank holidays, and current shift patterns will be from 08:00 AM to 16:00 PM and 10:00 AM to 18:00 PM. The role will involve working across all clinical areas depending on referrals received. Additionally, it may require working at other Sherwood Forest sites (Mansfield Community Hospital and Newark Hospital) as necessary.

The candidate should have a clear understanding of the Discharge to Assess process and sound knowledge of the discharge pathways. In addition, the candidate should also have awareness of social care and health processes. Full training will be provided for this role

  • Experience in managing patient flow.
  • Community resource experience
  • NHS social care or similar background
  • Care experience
  • Previous experience of admission/discharge processes
  • Computer skills
  • Knowledge of Trust policies

Qualifications

Essential

  • NVQ level 2 or equivalent
  • Good standard of general education including GCSE English or equivalent.
  • Audit Knowledge

Desirable

  • ECDL
  • NVQ in Promoting independence or equivalent in Care
  • NVQ in Health and Social Care

Further Training

Essential

  • Willing to undertake further training for personal development and for knowledge and skills development.
  • Mandatory training

Desirable

  • Understanding of the Delayed transfer of care act
  • Understanding of the National framework for NHS continuing Health Care
  • Knowledge of Medway
