Keyworker

The closing date is 04 December 2025

Job summary

Please note: This opportunity is open to internal candidates as a secondment. Before applying, please ensure you have discussed the post with your current line manager and obtained their agreement to support your release for the duration of the secondment. The countywide Kernow & Isles of Scilly Keyworking (KIOSK) team are recruiting to a Keyworker. We are passionate about children and young people who need access to high quality help. You will work alongside the keyworking team and Allied Health Professionals to work with families and young people to prevent unnecessary inpatient admission. The team prides itself on hearing the voice of the child and the family and developing a sense of their lived experience to understand needs. Keyworkers work hard to deliver person centred care alongside other services at times of crisis, bringing services together to work on shared goals and improved outcomes.

You will bring knowledge and understanding of the needs of children and young people who are autistic or with a learning disability and deteriorating mental health. In return you will be offered training to develop other areas of your practice in relation to the needs of this vulnerable group of young people. Working alongside other Allied Health Professionals, receive quality supervision and be given opportunities to develop the service alongside the Team Manager.

Main duties of the job

The Keyworker Role is a part of the NHS Improvement Long Term Plan to improve outcomes for children and young people with Autism and / or Learning Disabilities, who are either in a mental health inpatient setting or who are at risk of being admitted. More information on this initiative can be found at https://www.england.nhs.uk/learning-disabilities/care/children-young-people/keyworkers/

The role is unique in its development of a multi-agency commitment around the child to improve health, social and educational outcomes for the child and young person.

The Post holder will be expected to have excellent communication skills to enable them to live the journey with the young person. They will ensure that the child's needs are modelled across settings and that barriers to delivery are flagged with relevant agencies. The Keyworkers will be assigned to a small number of young people and work intensively with them and their families, under the supervision of a keyworking practitioner.

Successful candidates will be innovative and dynamic thinkers, who place the child and young person at the heart of decision making and ensure their voice is heard. Training for this role will build on the post holders understanding of autism, learning disabilities and how these relate to trauma and the risk of inpatient admissions. Continuing professional development opportunities are available and support will be given to deliver the role.

About us

We're an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people's physical and mental health. We also provide specialist support to people with dementia or a learning disability.

We are a people organisation and people matter to us. As part of the team, you'll help support the health and wellbeing of the people who live and visit this beautiful part of the UK.

Over 4,000 people make up the Trust. This includes doctors, nurses, therapists, plus admin and support staff.

We work in people's homes, in community clinics and bases. Some staff work from one of our 13 community hospitals. Our aspiration is to have great people, provide great care, be a great place to work and a great partner.

Just over 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increases the numbers of people who use our services.
