This is an exciting new opportunity to work with our Primary Care Dementia Practitioner Service. This is a countywide motivated innovative team who work closely with General Practitioners, community services and the third sector. We support people after their diagnosis and help people with dementia and their families live well. We offer support advise and help resolve any problems they may experience. We are always there when they need us.

Enthusiasm and kindness is required for this dynamic role for which we can offer and provide all appropriate training to be successful in this role.

The successful applicant willdeliver;

  • APHC (using remote health checks)
  • Monitoring role and referral/support with PCDP
  • Improving recording of health checks and dementia diagnosis
  • Case scenario outcomes to feed into service re-design

Main duties of the job

The post holder will be part of the PCDP team, contributing to supporting people with dementia in their own homes in the community. The team works within metal health services but has close links to General Practice and primary care colleagues.The post holder will provide a range of services including practical and emotional support to individual service users and their carers and complete the annual mental health check alongside the PCDP for GP.

Essential

  • NVQ Level 3 qualification in care or equivalent experience
  • GCSE Maths & English or equivalent qualification
  • Training to level 2 in Safeguarding children and young people: roles and competencies for Health Care staff

Experience/skills/knowledge

Essential

  • A basic understanding of current National Guidelines and Mental Health Act/Mental Health Capacity Act requirements
  • A basic understanding of CPA Process
  • IT literacy
