Job summary
As a Band 7 Community Nurse, you will be an exceptional autonomous practitioner, delivering advanced care in patients' homes and playing a key role in preventing avoidable hospital admissions. You will provide timely assessments, interventions, and clinical decisionmaking to support safe and effective patient flow.
You will be highly proficient across a wide range of advanced clinical skills, including IV therapy, palliative and endoflife care, complex wound management, catheter and continence care, diabetes management, PICC line care, medication administration, and drain tube management. Your expertise will enable you to manage complex cases independently and act as a senior clinical resource for the team.
In addition to your clinical responsibilities, you will provide strong leadership and support for Band 5 and Band 6 colleagues. This includes delivering training, offering supervision, and guiding staff in developing their clinical confidence and competence. You will lead by example, helping coordinate team activity and ensuring consistently high standards of safe, effective care.

Main duties of the job
As a Band 7 Community Nurse, you will provide highly skilled, autonomous care to patients in their homes, managing a wide range of complex and acute clinical needs. You will work closely with the Hospital @ Home team to support admission avoidance and ensure safe, effective patient flow across the system.
In this senior role, you will be expected to demonstrate advanced clinical proficiency across a broad scope of practice, including IV medication administration, palliative and endoflife care, complex wound management, catheter and continence care, diabetes management, PICC line care, medication administration, and drain tube management. You will be fully competent in these areas and able to act as a source of expert clinical guidance for the wider team.
You will make autonomous clinical decisions, undertake comprehensive assessments, and develop holistic care plans tailored to individual patient needs. As a Band 7, you will play a key leadership role--supporting the development of junior staff, providing supervision, and delivering highquality training to build capability and confidence across the team.
You will work collaboratively with Team Leaders and multidisciplinary colleagues, modelling clinical excellence and helping to drive innovation, high standards, and continuous improvement within the service.
