Heartbeat Primary Care CIC is looking for a driven and dedicated
GP's and ANP's to join our Clinical Services team to deliver high-quality care
across our hub sites.
These are key roles in the Better Access team. Post holders
are expected to deal with a wide range of health needs in a primary care
setting, delivering the highest standards of care. You will act within
professional boundaries, working autonomously without immediate support and
supervision, enabled by collaborative and supportive working relationships.
The post holder will deliver safe, effective clinical
decision-making and expert quality care, including assessment, diagnosis,
clinical management and evaluation with a patient-first approach and a
collaborative mindset.
Rates of Pay - Enhanced Access (Evenings and Weekends)
GP - £85.00 plus 12.07% annual leave allowance
ANP - £38.00 - £44.00 plus 12.07% annual leave allowance
Rates of Pay - In Hours (Between 8.00am - 6.00pm)
GP - £50.65 plus 12.07% annual leave allowance
ANP - £26.50 - £30.00 plus 12.07% annual leave allowance
Main duties of the job
Make professional, autonomous decisions in relation to
presenting problems, whether self-referred or referred to by other health care
workers.
Carry out a mixture of face to face or telephone
consultations
Clearly and accurately record consultations, including
safety netting advice, in a timely manner
Request appropriate investigations
Make safe and appropriate referrals to secondary care
Apply standards and codes of Practice set by the Royal
College of General Practice, GMC NMC HCPC , Department of Health and local NHS
trust.
Prescribe generically whenever appropriately and in
accordance with NICE guidance and Good Prescribing Practice (as defined by
British National Formulary guidance)
Provide clinical advice and support to colleagues.
Work within the multidisciplinary team to meet common goals.

The Enhanced Access service operates on behalf of Hambleton and Richmondshire PCNs and is delivered from a number of local General Practices and hospital.
The role is to cover a variety of shifts which are convenient to you. We offer in hours shifts (14.00-18.00hrs), and shifts on an evening (18.30 - 20.00hrs) and weekends (Sat 09.00-17.00hrs & Sun 09.00-13.00hrs).
We have many clinical services which require the expertise of a GP/ACP/ANP to complete the service delivery team

- Adapt communication methods to suit patients' individual needs, backgrounds, and understanding and perspectives.
- Maintain open, respectful, and professional communication within the team and with external partners.
- Represent patients needs and perspectives when appropriate.

- Deliver care aligned with NICE guidelines, local protocols, and evidence-based practice.
- Support audit cycles and use patient feedback to enhance services.
