Job summary
An exciting opportunity has arisen for full or part time Nurse to join South Waveney Primary Care Home team delivering high quality nursing care. The Team provides care at home to patients registered with GP surgeries in South Waveney. The PCH team works closely with GP's and other members of the multidisciplinary team to provide an excellent standard of person centered nursing care to adult patients in their own homes, residential care or clinic settings, supporting patients to self-manage whenever possible. The service uses SystmOne as a clinical IT system to support mobile working.
The successful candidate will be accountable for the effective management of a caseload, ensuring the delivery of high standards of evidence-based nursing care. Candidates will also demonstrate excellent organisational and communication skills .

Main duties of the job
The post offered includes full or part time options and sits in a multidisciplinary team providing community services 7 days per week between 07:00- 20:30 hours.
Applicants will be a registered nurse; Applications from newly qualified nurses will also be welcomed.
The role contains variety every day with a mixture of face-to-face clinical practice.
You will be given the opportunity to participate in appraisals and supporting junior members of the team.
Our band 5's also participate in a preceptoship if newly qualified or new to the community which aims to build confidence and competence in this transition. We also have clinical educators embedded within the teams who provide clinical support as well as a supervisor for all staff who will provide 1:1's and competency sign off.
The main duties of the role are: -
To work with other members of the multidisciplinary team to provide planned and unplanned nursing care.
To provide clinical case management for a delegated caseload of patients living in the community who require nursing care.
To work autonomously at a level appropriate to clinical competence and within scope of professional practice.
Contribute to the continuous evaluation of the service to ensure it is fit for purpose.
