22.5 hours per week.
The post
holder will work within a Public Health Locality Team, which includes Health Visitors and School Nurses, they will work with families mainly where there is a child aged up to 5 years.
The post holder will be responsible and accountable to Specialist Community Public Health Nurses/HV in the delivery of the DOH Healthy Child Program 0-5years.
The post holder will implement approaches to
promote and help prevent adverse effects on health and well-being
The post holder will deliver and evaluate
delegated and planned episodes of care and health promotion.

Main duties of the job
Working across a variety of settings the postholder will deliver planned episodes of care to families in their own homes. Support HV's and complete universal assessments in clinics and nurseries. Deliver public health messages and offer health education groups accross the locality.

effectiveness of specific activities as they proceed and modify
appropriately.

have knowledge of and act in accordance with local child protection policies
and procedures.

practice in accordance with the standards, protocols and policies which have
been agreed by Livewell Southwest, demonstrating knowledge of them and how to
access them.
