Job summary
Social Prescribing is a means of enabling GPs, nurses and other primary care professionals to refer people to non-clinical support. Recognising that peoples health is determined primarily by a range of social, economic and environmental factors. Social Prescribing seeks to address peoples needs in a holistic way. It aims to support individuals to take greater control of their own health and help them to move from a state of dependence to one of independence.
We monitor the impact of our interventions and provide proactive engagement and feedback to GP practices across Wrexham.
Main duties of the job
- Receive referrals from GPs and health professionals, supporting individuals to explore what matters to them and address issues affecting health and wellbeing. Help people make positive life changes through follow-up appointments and outreach. Refer individuals to community agents for practical solutions such as advice, adaptations, independent living skills, and social interaction. Provide high-quality information, promote healthy living, and connect people to a wide range of services. Develop social networks and local activities that sustain wellbeing long-term. Build strong relationships with partners, stakeholders, and local organizations. Engage face-to-face with service users to identify needs and interventions, working reactively and proactively to support older and vulnerable people. Facilitate community capacity, social inclusion, and sustainable caring roles through initiatives and support groups. Promote independence via living skills, adaptations, safeguards, technology, and housing advice. Deliver excellent customer service, maintain accurate records in compliance with data protection, and use IT effectively. Promote the Community Wellbeing Service, maintain client databases for outcome monitoring, and adhere to health and safety requirements. Support evaluation processes by preparing monthly reports, case studies, and examples of interventions.
