Healthcare Assistant - Community Nursing Network (Band 3)

Band 3: £25,760 - £27,476

37.5 hours per week (part time considered)

Location: North Kirklees & Greater Huddersfield

Main duties of the job

Locala is inviting applications from Healthcare Assistants who share our commitment to person-centred care and want to make a real difference to patients' lives.

As part of our Community Nursing Network, you'll work alongside qualified nurses to provide holistic, seamless care for housebound patients. Your role will include:

• Supporting nurses with assessments and care planning
• Delivering nursing-based care to prevent unnecessary hospital admissions
• Promoting self-care for patients with long-term conditions
• Contributing to health promotion and innovative service improvement projects
