Tŷ Hafan is a beautiful childrens hospice, located on the picturesque Vale of Glamorgan coastline. Covering the South, East, and West of Wales Tŷ Hafan offers a beautiful, newly refurbished working environment set in idyllic grounds a short distance from the metropolitan area of Cardiff.

We are pleased to announce a rare opportunity at Tŷ Hafan, for an experienced paediatric nurse to join our care team as a Band 6- Deputy Lead Nurse. We are looking for a dedicated nurse with passion and enthusiasm, able to lead others in providing holistic family centred care to our caseload of very special and inspiring children, young people, and their families.

Main duties of the job

Tŷ Hafans Deputy Lead Nurses are responsible for shift leadership, charged with assessing the childrens needs, effectively allocating and supervising staffing resources, ensuring the continuous safe delivery of care. The ability to make timely, intuitive and evidence-based clinical decisions is essential to this role. Support will be provided through working closely with our wider team of professionals. At Tŷ Hafan we promote individualised care planning, which goes beyond just clinical tasks. Our staff pride themselves on being able to make a lasting difference, through multiple opportunities we get to know the families well and support what is important to them.

As a Deputy Lead Nurse your responsibility will be to provide high quality leadership to the hospice-based care team, working as part of a multi-disciplinary team to ensure continuity of care. We are looking for a Deputy Lead Nurse with the confidence and skills to recognise deterioration. You should have the ability to prompt and support evidence-based clinical interventions in line with current protocols, guidelines, regulations and policies. Deputy lead nurses should be able to take timely actions including escalation, and reporting of identified concerns, ensuing onward referrals are made in the best interests of the child and their family.

To contribute to the provision of a quality 24hour service, being the nurse in charge of a span of dutyin the absence of a lead nurse or as required.

To provide expert paediatric palliative nursing care. To recognise deterioration and take timely action,including escalation where required.

To deliver nursing care in line with the All-Wales Palliative Care Standards for Children and YoungPeoples Specialised Healthcare Services and the End-of-Life care for infants, children and youngpeople with life-limiting conditions Nice Guidelines (2016).

To facilitate the provision of effective and efficient nursing care, participate in monitoring and evaluatingactivities in line with clinical governance. Ensuring the nursing care provided at Tŷ Hafan is maintain ata high standard.

To contribute to ongoing quality and service improvement projects as a transformational leader, toprovide clear guidance and support which empowers staff members to engage in changes in thepromotion of advancing care practices at Tŷ Hafan.

To support in the assessing, planning, implementing and evaluating of holistic care to children, andyoung persons with a range of complex life-shortening conditions.

When not in charge, to provide direct care and support for children independently or in collaborationwith colleagues.

To be responsible for the oversight of safe, effective and timely management and administration ofmedications.

To attend multidisciplinary team meetings and contribute as required, clearly articulating the needs ofchildren and families to ensure that palliative care delivery is needs led.

To promote effective discharge planning and hospital transfer, including high quality communicationwith parents and families, ensuring a safe transfer of services back into another setting is supported.

To support and be a positive role model for the implementation and use of data management systems(including RADAR).

To ensure that any clinical risks are identified and reported, and report any significant events inaccordance with policy, supporting investigation where necessary.

To maintain accurate, clear and contemporaneous records and documentation in line with Tŷ Hafansrecord keeping policy.

To undertake any other duties appropriate with the band and level of responsibility of this role for whichthe job holder has the necessary experience and/or training to complete.

To communicate effectively and timely with colleagues, children and families.

To be confident in communicating sensitive, complex information surrounding diagnosis, prognosis,treatment and bereavement.
