Job summary
We are looking for an experienced and
motivated Social Worker to join our Patient and Family Support Team, working
across our Little Hulton and Heald Green sites to support people living with a
life limiting illness, and those close to them.

The
successful candidate will be subject to a DBS Check at the level appropriate to
the role, the cost of which is covered by the hospice. Any post holder within
the organisation will be expected to undertake safeguarding training
appropriate to their role and adhere to safeguarding policies and procedures.
All staff must work in accordance with their statutory roles and
responsibilities in relation to safeguarding in accordance with the Working
Together to Safeguarding Children 2023, The Care Act 2014, and Prevent Duty
2015.

Main duties of the job
To provide specialist palliative care social work interventions to
people who access Moya Coles services.
Actively participate in service development and improvement. This will
include service evaluation, education and development relevant innovation programs
and research.
To act as a source of expert advice and support regarding social work
and safeguarding.
To deputise for the Patient and Family Support Team Leader/Advanced
Social Worker as and when required.
