Job responsibilities
JOB PURPOSE:
Be responsible, with others for planning, co-ordination, implementation
& evaluation of the Day Hospice service.
Work to increase the accessibility of Day Hospice to all demographics
and to widen the scope of referral criteria.
Support, supervise and develop, with others, the role of volunteers
within Day Hospice
Encourage patients self-ownership of their physical, emotional,
spiritual and social needs.
Be involved in quality, training, audit & research within the Day
Hospice
RESPONSIBILITIES:
Clinical
Make ongoing assessments of the total physical, emotional, spiritual,
social and practical needs of patients attending Day Hospice, including
planning, implementation and evaluation of patient care, considering the needs
of their family/carers
Provide direct clinical care appropriate to the role of Registered
Professional, as required
Achieve a relaxed and informal situation in which to enable the patient
and their family/carers adequate time to discuss anxieties and problems
Monitor individual programmes and be aware of changing needs
Work with the team to assist in planning and implementing diversional
therapy, utilising the skills of volunteers
Assist the multi-disciplinary team with the manual handling, transfer
and mobility of patients.
Maintain good communication with other members of the team for continued
patient care.
Identify any additional support the patient and their carers may need
and refer to Allied Health Professionals and liaise accordingly.
Assist in ensuring safe practices at all times in accordance with the
Health and Safety at Work Act maintaining high standards of care for patients.
Provide advice and support to the multi-disciplinary team members,
patients and their carers as required.
Promote health education with patients and their carers.
Participate in the multi-disciplinary team meeting including the planning,
delivery and evaluation of patient care.
Maintain accurate electronic patient records
Report and record all accidents, incidents and complaints.
Professional
Be a safe practitioner and work within the Nursing,
Midwifery Council Code of Professional Conduct (NMC)
Undertake continuing professional development
through the appraisal system
Work according to organisational policies, procedures
and guidelines
Maintain confidentiality.
Be familiar with fire, emergency and safety regulations, ensuring attendance
at statutory updates.
Maintain good working relationships with all members of the hospice
staff and volunteers.
Undertake any in-service training in line with Weston Hospicecares
policy.
Participate in Supervision
Education and Development
Participate in the orientation and development of new staff members and
volunteers.
Keep up to date with current practice and legislation.
Maintain and extend personal knowledge and expertise in all aspects of administration
and to share information.
Help implement and monitor policies and guidelines working within
legislative constraints.
As part of the multi-disciplinary team, to be involved in developing
standards of care
Take an active part in audit, monitoring and research programmes as
agreed with the Audit Group.
Other
Responsibilities
Maintain a responsible attitude towards economy and care of equipment
and other resources.
Maintain a smart, professional appearance utilising uniform at all times
in line with Hospice policy.

and Data Protection Act. Breaches of confidentiality in relation to
confidential, personal or sensitive data will result in disciplinary action,
which may include dismissal.
