Job summary
Provide holistic care and
support to children and young people with life threatening conditions and their
families. The post holder will be required to provide care across a range of
settings in the community, hospital and at the hospice and across localities
when required predominately to Babies, Children and Young People (BCYP) with
Long Term Ventilation (LTV) needs.
Main duties of the job
To work as a designated lead for
LTV care. Contribute to service and care developments under the clinical
direction of the locality Matron/LTV CNS to ensure provision of holistic care
and support to children with LTV care needs.
To teach, train and assess care
team staff on LTV practice through working alongside them in a range of
settings in the community and at the hospice.
The post holder will be required
to undertake a range of care tasks and duties, including reassessment of care
needs, care requiring the use of technology or specific techniques,
administration of medicines and communication and liaison duties.
To contribute to the delivery of
care on shift including end of life care if required.
Mentor new staff in relation to
LTV care.
The post holder will work in
collaboration with other services providing care and support for the child /
young person and family.
To
work as part of a team supporting a ventilator dependent child within the
hospice/ community setting and to continue that support independently within
the hospice and wider community.
