This is an exciting opportunity for a Registered Nurse passionate about palliative care to participate in Project ECHO supporting the care homes and supported living sector within Sheffield.
Project ECHO (Extension for Community Healthcare Outcomes) is a pioneering tele mentoring programme which was developed in the School of Medicine at the University of New Mexico (UNM). Featuring real life scenarios, case based learning and a variety of interactive methods, Project ECHO creates acommunity of practicethat supports service delivery, sharing of knowledge and support to staff.
Main duties of the job
St Lukes is a Superhub for Project ECHO in collaboration with Hospice UK and The Highland Hospice and leads on a strategic collaboration across South Yorkshire and Bassetlaw and also across Sheffield. We have trained a number of other organisations across the country to become ECHO hubs and we are also delivering a number of education programmes supporting the health and social care sector within Sheffield and across Yorkshire.
Using revolutionary video conferencing technology, ECHO enables the delivery of training and education direct from St Lukes to multiple sites where there is a training and education need to support service provision. This then creates a community of practice that supports service delivery, sharing of knowledge and support to staff in underserved areas.
The successful post holder will be able to demonstrate the ability to provide face to face palliative support and advice for residents, relatives and staff within care homes in Sheffield, the Supported Living Sectors as well as the wider primary health care team.
The successful post holder will also have the opportunity to develop their skills in the delivery of education.

Overall Purpose of the Role
Project ECHO is a pioneering method of education delivery of which St Lukes is at the forefront
along with Hospice UK and the Highland Hospice in Inverness enabling us to become trainers
of other organisations wishing to become Project ECHO Hubs. The St Lukes Project ECHO
team has also developed other work-streams providing interactive teleconferencing education
to organisations across Yorkshire and Humber and within Sheffield the care home sector
amongst others.
The community service provided by St Lukes aims to provide assessment, intervention and
monitoring of patients with progressive and life-threatening diseases enabling them to remain
at home which includes patients in residential, nursing home and other long stay settings for
as long as possible according to their preference.
The purpose of the SLH Band E Community Nurse within Project ECHO is to deliver
palliative and end of life care as part of the hospice multidisciplinary community services
providing nursing expertise, delivering specialist skills in patient centred holistic assessment,
symptom management, and effective clinical and advanced care planning. The role also has
an element of participating in education and training both internally and externally to the
organisation. The post holder will work under the direct clinical leadership of advanced
practitioner level community nurses but will be responsible to the Project ECHO Team Lead
and Head of the integrated Community team.
The post holder will undertake a role combining face to face assessment and support of
patients referred for community specialist palliative care support in care homes, dementia
units and supported living settings, supporting residents, relatives, care home staff and the
wider primary health care team to enable residents to remain in their preferred place of care
avoiding unwanted and unnecessary hospital admissions.
Also, in combination with the clinical role using a combination of technology and face to face
mentoring, the post holder will have the opportunity participate in a model of education and
training designed to optimise access to education for underserved areas such as care homes
in line with Health Education Yorkshire and Humber End of Life Care Learning Outcomes.
The post holder will have the opportunity to develop skills in delivering education sessions as
well as develop skills in facilitating education sessions using the principles of the Project ECHO
model under the supervision of the Project ECHO Team Lead.
The post holder will work to contribute to the community focused services delivered by the
hospice to combine national and local priorities for end of life care.
This will include achieving priorities that include reduction in the number of avoidable hospital
admissions and facilitating choice regarding place of care.
This will require a responsive and flexible service with Nurse led assessment contributing to
the wider multi-professional service in an efficient, effective and joined up way to ensure
person-centred high-quality care and a coordinated responsive hospice service.
