Job summary

One of our team of nurses has an exciting opportunity to travel for six months in 2026 and we are looking for someone to join us while she is away.

The role is from the end of January to the end of July, preferably working Monday, Tuesday and Wednesday.

Main duties of the job

You will provide primary care nursing covering a range of clinical duties including wound care, immunisations, chronic disease management (e.g., diabetes, asthma), health screening, and health promotion for all ages.

Your key responsibilities also include patient education, maintaining records, working alongside GPs, and running specialist clinics.

We are a semi rural Systmone practice based in the town of Tadcaster which is between Leeds and York.

Our team works closely together to provide care for our 9400 patients.

Job description

Job responsibilities

Assess the nursing needs of patients and provid a quality nursing service based on research evidence and practice within NMC guidelines.

Provide appropriate care/treatment in conjunction with the GP, Nurse Practitioner, or independently, as appropriate, and according to practice policy, guidelines, and protocols.

Provide care to patients with chronic diseases including diabetes, asthma, and COPD, in addition to undertaking screening activities such as cervical cytology.

Refer patients to other medical and nursing colleagues where appropriate.

Maintain computerised patient notes in the process of assessment of need and delivery of care. Evaluate the outcome of that care and makes changes to the care plan for individual patients.

Monitor, contribute, and implement evidence-based guidelines within clinical practice with the emphasis on a nursing and a holistic approach.

Implement, monitor and evaluate all NICE guidance within the practice.

Act as chaperone support if required by doctors/nurses.

Receive and transmit information on the telephone to the agreed practice standard, with particular reference to confidentiality.

Adherence to the requirements of the Quality & Outcomes Framework (QOF)

Undertake other clinical activities identified by the GPs and Senior Nurse in accordance with written protocols.

Organise their own work schedule within the constraints agreed by the GPs and Practice Manager.

Keep up to date on nursing developments and changes.

Be receptive and open to the implementation of new ideas and the change process.
