PCN Clinical Pharmacist

Job responsibilities

Job Purpose

To support a Primary Care Network (PCN) within Leeds in all aspects of medicines optimisation.

The post holder will work within their clinical competencies as part of a multidisciplinary team to provide expertise in clinical medicines optimisation across the PCN. This will include medication reviews with direct patient contact and may include contributing to:

Management of long term conditions

Management of medicines on transfer of care

Reviewing systems for safer prescribing

Contributing to repeat prescription authorisations and reauthorisation.

Addressing both the public health and social care needs of patients

Contributing to achievement of PCN DES contract, QOF and locally commissioned quality improvement schemes.

Undertaking clinical audit

Developing, promoting and implementing the high quality, evidence based and cost effective use of medicines in primary care in a way that maximises the benefits of medicines to patients and service users and which minimises risks (clinical, legal and financial).

The specifics of the role will be worked up between the post-holder and the PCN.

The post holder will be supported by existing pharmacy professionals employed within the PCN and a GP clinical supervisor. They will also engage with other pharmacy colleagues in Leeds for peer support.

The post holder will be required to enrol on the Primary Care Pharmacy Education Pathway from CPPE, following which will be required to enrol on a prescribing course.

Qualifications

Essential

• Professional knowledge acquired through vocational masters degree in pharmacy (4 years) or equivalent + 1 year pre-registration training
• Demonstrates and understanding of, and conforms to, relevant standards of practice
• Willing to enrol onto the Primary Care Pharmacy Education Pathway from CPPE OR completed the course
• Follows professional and organisational policies/procedures relating to performance management
• Awareness of the breadth of common acute and long-terms conditions that are likely to be seen in general practice

Desirable

• Demonstrates ability to integrate general practice with community and hospital pharmacy teams and community groups

Professional registration

Essential

• Mandatory registration with the General Pharmaceutical Council

Desirable

• Membership Primary Care Pharmacy Association (PCPA)
• Membership of the Royal Pharmaceutical Society

Skills & Knowledge

Essential

• Knowledge of IT systems, including ability to use word processing skills, emails and the internet to create simple plans and reports
• Able to gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers

Desirable

• In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare
• Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities
• An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing. Analyse and interpret complex prescribing information and present information in a clear way
• Able to plan, manage, monitor and review general medicine optimisation issues in core areas for long term conditions
• Able to obtain and analyse complex technical information. Interpret data to draw conclusions
• Able to identify and resolve risk management issues according to policy/protocol
• Understand the principles of research governance
