Job summary
Purpose of the Role
The ACP will be a senior autonomous
clinician working collaboratively within the multidisciplinary team. The role
will support the practices skill-mix model to enhance access, improve
workflow, and allow GPs to focus on complex and continuity care. This post
forms part of our longer-term workforce planning to maintain stability and
sustainability beyond April 2026.
Main duties of the job

Independently assess, diagnose
and manage patients presenting with acute, chronic, and same-day conditions.
Request, interpret and act on
diagnostic investigations and imaging as appropriate.
Prescribe medication
independently (V300 qualification required) and optimise treatment plans in
line with best practice.
Contribute to the management of
long-term conditions and urgent care clinics.
Support triage, digital
consultation, and online form workflow.
Participate in quality
improvement, clinical audit, and significant event analysis.
Provide clinical supervision
and mentorship to junior clinicians and ARRS colleagues.
Engage in weekly supervision
and protected education sessions.

Thamesmead Medical Associates
(www.thamesmeadmedical.org) is a large, well-established GP training practice
in South East London with approximately 16,000 registered patients. The
practice is rated Good by the Care Quality Commission (CQC) and provides high-quality,
patient-centred care to a diverse local population.
We are a recognised training environment for GP registrars and Foundation
Doctors and have a broad multidisciplinary team including ARRS roles such as
pharmacists, paramedics, and social prescribing link workers. The practice
