Job summary

We are looking for an enthusiastic and compassionate Practice Nurse to join our friendly and supportive team at The Village Medical Centre in Wallasey for a one-year maternity cover contract.

The role will include the full range of general practice nursing duties such as immunisations, cytology, wound care, long-term condition management, health promotion and patient education. We are happy to support further training and will tailor the workload to reflect your skills and experience.

Main duties of the job

As a Practice Nurse at The Village Medical Centre, youll provide safe, evidence-based nursing care to our patients across a range of clinical areas. Your role will include managing long-term conditions such as asthma, COPD, hypertension and diabetes; delivering adult and childhood immunisations; undertaking cervical screening; and providing wound care, dressings and other treatment room services.

Youll play an active role in health promotion, patient education and supporting self-management. Working closely with our GPs, HCAs and wider primary care network, youll help ensure continuity of care and contribute to quality improvement initiatives within the practice.

The post holder will work as part of the nursing team at The Village Medical Centre to provide high-quality, evidence-based nursing care to the practice population. You will deliver a range of clinical services including immunisations, cervical cytology, chronic disease management, wound care, health screening and health promotion activities. The role will involve both treatment room and chronic disease management work and requires a proactive, adaptable and caring nurse who enjoys working collaboratively within a multidisciplinary primary care team.

Key Responsibilities

Clinical Practice

  • Provide general treatment room nursing including dressings, ear care, ECGs, blood pressure monitoring and phlebotomy (if trained).

  • Undertake adult and childhood immunisations and vaccinations in line with national guidance.

  • Perform cervical cytology screening.

  • Provide long-term condition reviews for patients with asthma, COPD, hypertension, diabetes and other chronic illnesses, working to evidence-based protocols.

  • Support patients in managing their conditions through health education and lifestyle advice.

  • Provide wound care and post-operative follow-up.

  • Assist with health checks, NHS Health Checks, and opportunistic screening.

  • Maintain accurate clinical records and ensure all data entry meets clinical governance standards.

  • Support infection prevention and control measures within the practice.

Professional & Team Responsibilities

  • Work collaboratively with GPs, nursing colleagues, healthcare assistants and administrative staff to deliver seamless, coordinated care.

  • Contribute to practice audits, quality improvement initiatives and the achievement of QOF and enhanced services.

  • Maintain registration with the NMC and ensure continuing professional development (CPD) in line with revalidation requirements.

  • Follow all practice protocols and policies, including safeguarding, confidentiality, and information governance.

Administrative & Organisational Duties

  • Record and code consultations accurately using EMIS clinical system.

  • Support the monitoring and recall systems for chronic disease reviews, immunisations and screening.

  • Participate in annual appraisals, training and development opportunities.

  • Contribute to clinical governance activities such as audits, policy reviews and significant event discussions.
