Job summary
We are looking for a full time Advanced Nurse Practitioner to join
our triage team within the practice. We
are embracing the new general practice access model, seeking to develop new
pathways and triage processes, to support our patient population and help them
gain access to the right service as soon as possible. You'll be working alongside experienced GPs,
Care Navigators and other clinical staff such as Pharmacists and experienced Practice
Nurses, using your assessment and diagnostic skills, supported by triaging software
and clinical supervision.

Main duties of the job
Working within our triage team, you'll be a core team member, supported by a dedicated GP to assess and treat patients with a wide range of minor ailments, illness and other conditions which require same day assessment. You'll help support the care navigation team, by providing feedback as well as developing protocols, supporting training, development and audit. You'll help support the introduction of triaging software within the practice, as well as working with other ANP's and clinicians within the Severn Health PCN to develop standard approaches to managing same day demand for minor illness.
You'll be confident to work as a independent practitioner, with a prescribing qualification and you will have gained a master levels qualification to enable you to practice at this level.

training practice, with a number of Registrars, Medical Students, Nursing
Associate placements as well as Apprentices.

Working alongside and collaboratively with GPs and other practice colleagues, to clinically assess, diagnose, treat, and evaluate care for patients of Five Valleys Medical Practice, Gloucestershire. Responsibilities:
- Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
- Clinically examine and assess patient needs and plan clinical care accordingly.
- Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.
- Prescribe and review medication for therapeutic effectiveness.
- Work with patients to support compliance with and adherence to prescribed treatments.
- Provide information and advice on prescribed, or over-the-counter medication on medication regimes, side-effects, and interactions.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
