Job summary
An exciting opportunity to join H&F Partnership as a Probation Services Nurse to deliver healthcare interventions in probation settings, helping to address health inequalities and improve outcomes for individuals involved in the criminal justice system. The role involves conducting health assessments, providing health education, identifying unmet healthcare needs, and coordinating referrals to relevant services.
INTERVIEWING will commence from 20th July 2026.
Main duties of the job
The following are the core responsibilities of the Probation Office Nurse.
- act as main health contact for patients on the probation caseload.
- complete an initial health screening assessment/template.
- complete ADHD screening assessment where necessary.
- ensure patients are supported and appropriately signposted to relevant primary care services.
- develop, implement and embed health promotion and wellbeing programmes.
- identify, manage and support patients at risk of developing long-term conditions, helping prevent adverse effects on their health.
- oversee chronic disease clinics, delivering care as required and referring patients to secondary/specialist services where appropriate.
- maintain accurate clinical records in line with current legislation and professional standards.
- ensure Read codes are used effectively.
- support the appropriate maintenance and effective use of chronic disease registers.
- act as a chaperone where required.
- prioritise health issues and intervene appropriately.
- recognise, assess and refer patients presenting with mental health needs.
- liaise with external services and agencies to ensure patients, including vulnerable patients, are appropriately supported.
- delegate clinical responsibilities appropriately, ensuring safe practice and that tasks remain within the individuals scope of competence.
- understand practice and local policies relating to substance misuse and addictive behaviours, referring patients appropriately.
- deliver opportunistic health promotion.
