Job summary
An exciting opportunity has arisen to work as a community matron in a collaborative role. The role is shared between Leeds Community Healthcare (LCH) and Woodsley and Holt Park Primary Care Network (WHPCN).
The successful candidate will work both with LCH and WHPCN to help innovate the way we deliver healthcare in primary care and the community, taking a lead role in transforming and improving service delivery for our patients.
The working week will be split between days working for LCH and WHPCN separately. The exact split of these days will be agreed between both organisations and the successful candidate after their interview.
Your contract will be permanent with the PCN and the LCH days will be a secondment for an initial 12 month period. After this we hope to demonstrate the success of the collaboration to shape the role moving forward.
The PCN opportunity:
Woodsley and Holt Park PCN cover a large diverse population of approximately 110000 patients in the West of Leeds. Although we are two PCNs we essentially work as one with an over-arching leadership team which is accountable to both PCNs - our PCN combined comprises of ten GP practices.

Main duties of the job
The LCH opportunity:
The post holder will be responsible for delivering and leading Advanced clinical care to their own complex caseload, Working within the 4 Pillars of Advanced Clinical Practice: Education, Clinical Leadership, Clinical Skills, and Research
For providing leadership to Caseload Clusters within their Neighbourhood Team.
Support / oversee pre-booking and the allocation of visits to promote the right skills, at the right time, in the right place
Utilising their Advanced Specialist Skills to assess, plan, deliver and evaluate patient centred programmes of care ensuring that they practice within their sphere of competence and knowledge.
Will provide clinical support and supervision to Neighbourhood Team members.
Will work to professional and regulatory body codes, standards and guidance at all times ensuring that their practice is grounded in evidence based theoretical and practical knowledge.
Be responsible for developing new and innovative clinical practices responding to emerging knowledge and techniques.
Experience of managing sub acutely ill patients at Advanced Practice Level.

About us
We are a dynamic, motivated and enthusiastic PCN who have successfully introduced large teams of social prescribers, physiotherapists, pharmacists and pharmacy technicians delivering high quality healthcare across our PCN. We have also just recruited podiatrists, dieticians, occupational therapists, care coordinators as well as health & wellbeing coaches. We now have over 50 additional roles working across our PCN providing high quality healthcare for our patients.
Our PCN is developing a service for our frail patients called the Ageing Well Team. This multi-professional team is predominantly a home visiting service aiming to providing high quality proactive care for this patient cohort who have complex needs. We have already recruited a community matron, x2 occupational therapists, a senior pharmacist and x2 care co-ordinators into the team. In this recruitment cycle we hope to add another community matron to this expanding dynamic service.
The successful applicant will have the ability to work independently as well as being able to lead a team, the ability to work under pressure is essential with excellent communication.
The PCN uses System One and EMIS as their clinical systems. Although the successful candidate will ideally have previous experience of working these systems, training can be provided.
