Main duties of the job
In this role, you will deliver a blend of long-term condition management and general treatment room care, supporting patients with a wide range of clinical needs. You will carry out routine nursing assessments, provide health promotion advice, and work with patients to help them manage their conditions effectively. You will also contribute to preventative care such as vaccinations, wound management and screening activity, helping us maintain high standards of safe and efficient clinical practice.
Working closely with GPs, ACPs, pharmacists and the wider team, you will play an important role in coordinating patient care and ensuring smooth patient pathways. You will support quality improvement activity, contribute to a positive team culture and help develop services that improve patient experience. 3. Job PurposeThe Practice Nurse will deliver high-quality nursing care within a primary care environment, providing a wide range of treatment room and long-term condition services. The postholder will work as part of the multidisciplinary team to promote health, manage chronic diseases, support preventative care and contribute to the overall quality and safety of the practices clinical services.
4. Key ResponsibilitiesClinical Duties

Undertake chronic disease management reviews, for example diabetes, asthma, COPD and hypertension, in line with current guidelines.

Provide treatment room services, including wound care, dressings, ear irrigation, cervical screening, immunisations and injections.

Deliver childhood, adult and travel vaccinations in accordance with recommended schedules.

Support womens health services such as cervical cytology and contraceptive advice within competence.

Provide health promotion and lifestyle advice to support self-management and disease prevention.

Triage and manage minor illnesses or minor injuries within scope of practice.

Undertake NHS Health Checks and other preventative screening where required.
