Care Coordinator

We are looking for a proactive and organised Care Coordinator to join our team and play a key role in supporting patient care. This is an ideal opportunity for someone who enjoys administrative work and wants to make a real difference to patient outcomes within a busy GP practice.

In this role, you will manage a range of essential administrative tasks, including running clinical searches, processing patient registrations and deductions, and contacting patients from targeted lists. You will also take ownership of patient recall systems for long-term conditions, ensuring patients are invited for regular reviews and receive timely, coordinated care from the appropriate healthcare professionals.

You will act as an important link between patients and the clinical team, helping to keep records accurate, workflows efficient, and care pathways well organised. Your work will directly support improved patient engagement, continuity of care, and overall practice performance.

We are looking for someone who is detail-focused, confident using clinical systems, and comfortable communicating with patients over the phone. If you are reliable, adaptable, and motivated to contribute to high-quality patient care, this role offers a rewarding opportunity to develop your skills in primary care.

Main duties of the job

You will provide day-to-day administrative support to ensure patients are contacted, booked and followed up appropriately. The role involves working with patient lists, coordinating routine reviews, and helping maintain accurate practice records.

You will regularly contact patients by phone, support the smooth flow of appointments, and help ensure no patient is missed from essential care. The post also requires close working with clinical and reception teams to keep systems organised and up to date, contributing to a safe and efficient service.

Main Purpose of the RoleTo support the delivery of high-quality patient care through effective administrative coordination, patient engagement, and management of recall systems, ensuring patients receive timely and appropriate care.

Key Responsibilities

  • Manage patient recall systems for long-term conditions, ensuring patients are invited and booked for routine reviews in line with clinical targets
