Job summary

We are looking for an Advanced Clinical Practitioner to join our established Clinical Team.

Applicants must have a passion for providing excellent, professional, and effective care whilst ensuring our patients feel safe and respected.

Applications are invited from healthcare professionals with a Nursing, Pharmacist, or Paramedic background.

Main duties of the job

The post holder will act within their professional boundaries, providing care for the presenting patient.

They will demonstrate safe, clinical decision-making, including assessment and diagnostic skills for patients within the practice.

Independently assess, differentially diagnose, plan, implement and evaluate individualised treatment plans for patients, resulting in safe appropriate management, referral, or discharge without direct medical support on site.

Prescribe independently (where legally permitted) within scope of competence and within the Royal Pharmaceutical Society Competency Framework, national and locally adapted guidelines, and best available evidence for a range of urgent/acute presentations appropriate to patients need and desired therapeutic outcomes.

Maintain effective communication with the immediate and wider health care teams to promote seamless care/facilitate effective ongoing care both in all settings.

The Mount View Practice is a forward thinking, patient focused, professional and friendly team looking after approximately 12000 patients in Fleetwood.

We consist of 5 GP Partners and a Salaried GP, a Practice Paramedic, Acute & Therapy Mental Health Team, Advanced Nurse Practitioners, Pharmacist & Tech, Practice Nurses and Healthcare Assistants and a dedicated team of administrative professionals.

We are part of Fleetwood Primary Care Network and supported by an MDT ARRs Team.

Job description

Job responsibilities

Nurses / Pharmacists / Paramedics

Participate in all treatment and preventative healthcare services in the Practices as delegated and agreed by the supervising GP(s).

Consult and refer patients to physicians, medical specialists and other health professionals as indicated.

Provide direct clinical care for patients presenting with undifferentiated, undiagnosed problems by utilising history-taking, physical examinations, and clinical decision-making skills to establish a working diagnosis and management plan in partnership with the patient (and their carers where applicable) during surgery consultations and home visits.

Recommend and explain appropriate diagnostic tests and treatment.

Request and interpret results of laboratory investigations when necessary.

Perform specialised diagnostic physical exams and treatment procedures.

Instruct and educate patients in preventative health care.

Order laboratory tests as required and agreed under supervising doctor(s).

Fully document all aspects of patient care and complete all required paperwork for legal and administrative purposes.

Formulate diagnoses and treatment plans, in consultation with or referring to Supervising doctor(s) as appropriate.

Communicate, when necessary, with colleagues in the Practice, PCN and hospital specialists to discuss or refer specific patients, plan and co-ordinate activities or exchange information to improve the quality of patient care.

Deal regularly with hospitals, consultants and other health care providers, and other community organisations to refer/consult on specific cases.

Provide health/disease promotion and prevention advice, alongside analysing and actioning diagnostic test results.

Develop integrated patient-centred care through appropriate Working with the wider primary care multi-disciplinary team and social care networks.

Utilise clinical guidelines and promote evidence-based practice and partake in clinical audits, significant event reviews and other research and analysis tasks.

Undertake home visits when required.

Pharmacists Only

Help achieve efficiencies and effectiveness within the prescribing activities of the practice

Promote high quality cost-effective and evidence-based use of medicines

Work within the Pharmacy Team, to respond to medication-related queries generated by patients, GPs, other health care professionals and staff

Work within the Pharmacy Team to develop and ensure a safe efficient medicines optimisation programme including repeat prescribing, medication review and medicines reconciliation services

Undertake clinical medication reviews either face to face or via telephone support on identified stratified-risk patients that may include residents in care homes, in their own, with long-term conditions, and/or learning disabilities

Identify and produce recommendations for medication optimisation for the GPs and other multi-disciplinary team members

  • Registered healthcare practitioner with an appropriate regulatory body (i.e., NMC, HCPC)
  • Completed masters degree qualification (or equivalent prior to MSc being available e.g., Nurse Practitioner Degree, OSCEs and non-medical prescribing)
  • Independent prescriber
  • Advanced clinical practice skills and clinical treatment skills
  • Evidence of substantial clinical practice at advanced level
  • Evidence of competence within all core ACP competencies

Desirable

  • Mentor/teaching qualification

Experience

  • Experience in managing acute problems.
  • Clinical diagnostic skills including the ability to interpret radiology and laboratory investigations.
