Job responsibilities
- Conduct comprehensive clinical medication reviews for patients with complex needs (e.g. diabetes, hypertension, COPD, asthma, heart failure).
- Deliver face-to-face and telephone consultations.
- Respond to medication queries from patients, carers, and healthcare professionals.
- Participate in clinical audits and quality improvement projects.
- Manage clinical correspondence and monitor outcomes, liaising with secondary care and community services.
- Maintain accurate records of clinical interventions.
- Stay up to date with developments in primary care medicine.
- Oversee repeat prescription systems, acute requests, and safer prescribing initiatives.

Qualifications
Essential
- Masters degree in Pharmacy (MPharm) or equivalent.
- Registered with the General Pharmaceutical Council (GPhC).
- Member of the Royal Pharmaceutical Society (RPS).
- Independent Prescribing qualification.
- Enrolled in or completed an approved 18-month Clinical Pharmacist training pathway (e.g., CPPE).
Experience
Essential
- Experience managing chronic diseases and conducting clinical medication reviews, especially for complex patients.
- Excellent communication, interpersonal, and organisational skills.
- Ability to work independently and as part of a team.
Desirable
- Postgraduate qualifications (e.g. Postgraduate Diploma in clinical, community, or therapeutic pharmacy).
- Familiarity with SystmOne clinical system.
- Additional qualifications in chronic disease management.
- Knowledge of QOF, IIF, and PCN-directed services.
- Previous experience in general practice.
