Job summary

This is an exciting time for General Practice and our expndin PCN. All practices in the country have come together to work in local PCNs. These are groups of GP practices working in partnership to support their local population. Enfield South West PCN is looking for a Paramedic Practitioner, AFC Band 7-8A, to join our growing, friendly and forward thinking team to manage (with support) the acute home visits of registered patients and work with our Duty Doctors and Practice Nurses.

Main duties of the job

As a Paramedic in General Practice, you will work as part of a multi-disciplinary clinical team across 6 practices in our PCN. You will provide a high-quality, cost-effective service to the patients; diagnosing / prescribing treatments while working within defined procedures and protocols.

Typical patients will present with a wide range of minor illness, acute illness and chronic disease management needs, the post holder will be responsible for seeing patients in the practice and visiting patients in their home environment.

This Paramedic Practitioner post is designed to support our practices, in providing quality patient centred care to these groups, with guidance provided by their clinical teams.

About us

Our PCN serves a population of over 49,000 patients across the South West of the Borough of Enfield. There is a wide socio-economic and demographic population served within this area, which will provide an interesting and varied patient caseload. There are six GP practices, so adaptability and mobility are essential.

You will be joining a growing and developing team of clinicians, integrated with our current practices workforce, including Pharmacists, Physician Associate, Care Coordinator and Social Prescribers. We are very excited to have these new roles join our team and for us to co-create new ways of working.

Main Job Responsibilities ( Full Job Description attached) :

1. Work as part of a multi-disciplinary team (MDT) within the PCN.

2. Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.

3. Advise patients on general healthcare and promote self- management where appropriate, including signposting patients to the PCNs social prescribing service, and where appropriate, other community or voluntary services.

4. Perform specialist health checks and reviews within their scope of practice and in line with local and national guidance.

5. Perform and interpret ECGs and other investigatory procedures, as required

6. Undertake the collection of pathological specimens including intravenous blood samples, swabs, and other samples within their scope of practice, and within line of local and national guidance.

7. Support the delivery of anticipatory care plans and lead certain services (e.g. monitoring blood pressure and diabetes risk of elderly patients).

8. Provide an alternative model to urgent and same day care / GP home visiting, for the PCN.

9. Communicate at all levels across organisations ensuring that an effective, person-centered service is delivered.

10. Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required.

11. Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.

12. Communicate effectively with patients, and where appropriate family members and their Carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings, and treatment choices.

Qualifications

Essential

  • Educated to degree/diploma level in Para-medicine or equivalent experience
  • Registered with the Health and Care Professions Council (HCPC)
  • Completed two-year Consolidation of Learning period as a newly qualified paramedic
  • Have a further three years experience as a band 6 (or equivalent) paramedic

Desirable

  • Post graduate diploma in either Minor Illness or A&E care or equivalent.
  • Masters degree level in a relevant area of expertise
  • Have the capabilities of advanced clinical practice set out in section one of the Multi-professional Framework for Advanced Clinical Practice in England

Experience

Essential

  • Substantial experience in either primary care, A&E or an acute setting.
  • Minor Illness / Triage care
  • Home visit assessment of patients
  • Previous experience of Autonomous practice in a community or practice setting

Desirable

  • Management of chronic disease conditions in general practice, i.e. Diabetes, Asthma/COPD, Heart Disease, Dementia
  • Previous experience in developing service provision
