Job summary

The post-holder will maintain a caseload of patients, ensuring the highest quality of care for all registered pateints. A variety of skills with be required to deal with a large range of health needs across a primary care setting.

Main duties of the job

Develop and manage a medicines management plan and deliver patient services as determined by practice policy and local and national guidance. Develop the medicines management team, including delivery of training, in order to maximise cost-effective prescribing and improve the quality of patient care.

Newton Drive Health Centre has over 12,500 patients.

We have a team of 3 Partner GP's, 4 Salaried GP's, a Nurse Practitioner, 3 Practice Nurses, a HCA and Phlebotomy Team and various ARRS roles.

Job responsibilities

Clinical Responsibilities

  • In accordance with the Practice rota, with prior agreement, the post holder will undertake various duties including surgery consultations, telephone consultations and queries, home visits, checking and signing repeat and acute prescription requests as well as dealing with paperwork and correspondence
  • To make professional and autonomous decisions in relation to presenting problems with regards to prescribing and treatment in line with local formularies whenever it is clinically appropriate
  • Assessing the health care needs of patients with undifferentiated and undiagnosed problems
  • Admitting or referring patients to other care providers as necessary in line with local procedures and pathways
  • To screen patients for disease risk factors and early signs of illness
  • To maintain the clinical record of patients using EMIS Web and to keep clear and contemporaneous consultation notes to agreed standards
  • To participate in multidisciplinary clinical meetings with external organisations and within the practice
  • To maintain own clinical and professional competence and be responsible for own continuous professional development
  • To participate and suggest clinical audits to benefit priority areas such as QOF, NICE Guidance, GP Plus or other agreed targeted areas
  • To work effectively across the neighbourhood HUB and other community / external services
  • In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working in primary care

To have an awareness of all relevant practice policies and procedures e.g. prescribing, confidentiality, data protection, health and safety and infection control

Attending training and events organised by the practice and/or other agencies, where appropriate

Essential

  • + Up to date and fit to practice safely
  • + Experience of Primary Care or working with GP Practices
  • + Evidence of achievement of foundation competencies
