CLINICAL PHARMACIST JOB DESCRIPTION
JOB TITLE: CLINICAL PHARMACIST
REPORTS TO: GP PRESCRIBING LEAD/STRATEGIC MANAGER
WAGE: Band 7 - 8A Depending on experience
HOURS: Minimum 30 to 37.5 hours per week without Primary Care Pharmacist Qualification.
JOB SUMMARY:
The post holder will be a pharmacist, who acts within their professional boundaries, alongside another PCN pharmacist and 4 pharmacy technicians across our three practices; Honiton Surgery, Coleridge Medical Centre and Sid Valley Practice. They will work with our GPs and other clinicians to support prescribing and review of medication in accordance with the PCN DES. They will support Community Pharmacies and Care Homes to improve standards of medicines management for all patients in our PCN and strive to improve cross sector working.
JOB RESPONSIBILITIES:
Carry out structured medication reviews for patients identified as priority through the PCN DES. This will be provided by face to face and phone appointments and may also involve visiting people in their own homes or in care homes. Although the candidate does not need to be a non-medical prescriber at time of application, they must be willing to work towards this goal and will be supported to achieve this following completion of the CPPE Primary Care Pharmacist programme.
Carry out medicines reconciliations for patients across care boundaries to ensure that our patients get the right medicines in a timely manner with the correct drug monitoring. This will involve liaising with colleagues in secondary care and possibly accessing further support through the community pharmacy discharge medicines scheme or referral to the community services pharmacy team
- Answer prescription and medication queries from patients and staff across the health network.
- Present and lead improvement projects on PCN wide policies and procedures.
- Contribute to PCN prescribing committee meetings, practice prescribing meetings & GP educational meetings.
- Support the GP prescribing leads with implementing change, maintaining standards and achieving prescribing targets e,g, those set through the Devon Medicines Optimisation Scheme.
Liaise with community pharmacy colleagues to align cross sector initiatives e.g. GPCPCS, eRD.
Complete and communicate audits to ensure a culture of continual quality improvement
Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.
Support the PCN pharmacy technicians where necessary
Document all work in a thorough and timely way in the practice patient management system (SystmOne)
Advise and support with queries regarding medication storage and transportation including vaccines
CONFIDENTIALITY:
- In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately
- In the performance of the duties outlined in this Job Description, the post-holder may have access to confidential information relating to patients and their carers, Surgery staff and other healthcare workers. They may also have access to information relating to the Surgery as a business organisation. All such information from any source is to be regarded as strictly confidential
- Information relating to patients, carers, colleagues, other healthcare workers or the business of the Surgery may only be divulged to authorised persons in accordance with the Surgery policies and procedures relating to confidentiality and the protection of personal and sensitive data
HEALTH & SAFETY:
The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the surgery Health & Safety Policy, the surgery Health & Safety Manual, and the surgery Infection Control policy and published procedures. This will include:
- Using personal security systems within the workplace according to Surgery guidelines
- Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks
- Making effective use of training to update knowledge and skills
- Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards
- Actively reporting of health and safety hazards and infection hazards immediately when recognised
- Keeping own work areas and general / patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holders role
- Undertaking periodic infection control training (minimum annually)
- Reporting potential risks identified

PERSONAL/PROFESSIONAL DEVELOPMENT:
The post-holder will participate in the CPPE primary care pharmacist training programme if not already completed, They will also complete other training as required by the PCN or individual GP practices.
They will participate in an annual individual performance review, including taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work

The post-holder will strive to maintain quality within the PCN, and will:
- Alert other team members to issues of quality and risk
- Assess own performance and take accountability for own actions, either directly or under supervision
- Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance
- Work effectively with individuals in other agencies to meet patients needs

The post-holder should recognise the importance of effective communication within the team and will strive to:

CONTRIBUTION TO THE IMPLEMENTATION OF SERVICES:
The post-holder will:
- Apply PCN and Surgery policies, standards and guidance
- Discuss with other members of the team how the policies, standards and guidelines will affect own work
- Participate in audit where appropriate
