Practice Nurse

Job summary

We are looking for a Practice Nurse with experience in Primary Care to join our Nursing Team delivering high-quality patient care to our population.

The post is part-time, up to 3 days per week, on a fixed term basis but with an opportunity to become permanent for the right candidate.

Main duties of the job

  • Working as part of the Practice Nursing Team in caring for our Practice population
  • Providing assessments, screening, treatment services, and health education advice
  • Vaccination and immunisation
  • Provide appropriate follow ups for patients
  • Management of chronic disease
  • Ensure accurate notes are recorded
  • Attend and participate in practice meetings as required
  • Undertake administrative procedures including stock checks, fridge monitoring, vaccine ordering, lab links, tasks etc.
  • Participate in audits

About us

We are a large Practice of 15,150 patients in Sunderland North. The Practice is part of the Deep End Network delivering care to the most deprived in the area.

Our team consists of 3 GP Partners, 6 Salaried GPs, 5 ANPs, 2 PNs, 1 NA & 1 TNA, 1 HCA, 1 Phlebotomist, 1 Clinical Pharmacist, 1 Pharmacy Technician, the PCN Team and a large Admin Team.

In addition to the core responsibilities outlined in the job description, the Practice Nurse may be requested to;

1.Develop, review and implement nursing protocols in conjunction with Lead Practice Nurse/National Nursing Lead

2. Support all members of the nursing team, providing guidance when necessary

3. Participate in local initiatives to enhance service delivery and patient care

4. Support and participate in shared learning within the practice in order to improve patient care

5. Continually review clinical practices, responding to national policies and initiatives where appropriate

6. Participate in the review of significant and near-miss events applying a structured approach

7. Participate in the planning and implementation of changes withing the nursing team to enhance the provision of services to patients

8. Support the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to existing legislation
