Job summary

The Advanced Nurse Practitioner (ANP) will work autonomously within the multidisciplinary team to provide high-quality, evidence-based care to patients across the lifespan. The postholder will assess, diagnose, manage, and treat patients presenting with acute, chronic, and complex health problems, referring to or liaising with other professionals as appropriate.

The ANP will act as a senior clinical leader within the Practice, supporting service delivery, clinical governance, and quality improvement initiatives aligned with NHS and local PCN priorities.

Main duties of the job

  • Provide expert, autonomous clinical assessment, diagnosis, and treatment for patients presenting with a wide range of conditions.

    • Initiate and interpret diagnostic tests and investigations.

    • Prescribe medicines safely and appropriately under the NMC prescribing standards. Provide long-term condition management e.g., diabetes, COPD, asthma, hypertension. Deliver urgent care and triage same-day appointments.

    • Undertake home visits or care home reviews as required.

    • Promote self-care, prevention, and health education.

    • Identify, manage, and refer safeguarding concerns appropriately.

  • Provide expert, autonomous clinical assessment, diagnosis, and treatment for patients presenting with a wide range of conditions.

  • Contribute to CQC compliance and preparation for inspections.
