Job responsibilities

Clinical Care Delivery

• Provide patients the choice of direct or remote access to an ACP/ANP, both in general practice, via digital media, telephone or home setting for the general practice population.
• Make professionally autonomous decisions for which he/she is accountable.
• Provide a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision making, to establish a diagnosis and management plan.
• Instigate necessary invasive and non-invasive diagnostic tests or investigation and interpret finding/ reports.
• To prescribe safe, effective and appropriate medication as defined by current legislative framework.
• Provide safe, evidence-based, cost effective, individualised patient care.
• Offer a holistic service to patients and their families, developing where appropriate an ongoing plan of care/ support, with an emphasis on prevention and self-care.
• Promote health and well-being through the use of health promotion, health education, screening, and therapeutic communication skills.
• Refer patients directly to other services/ agencies as appropriate.
• Participate in developing and delivery specialist primary care services such as long term disease management and ensure relevant frameworks are being delivered.
• Work with multi-disciplinary teams within the practice and across the wider ICB to promote integrated and seamless pathways of care.
• Contribute to the practice achieving it quality targets to sustain the high standards of patient care and service delivery.
• Participate in identification of community health needs and develop patient/family-centred strategies to address them in liaison with appropriate community health professionals and agencies.
• Be aware of and contribute to planning and delivery of practice-based commissioning.

Professional Role:

• Promote evidence based practice through the use of the latest research-based guidelines and the development of practice based research.
• Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review.
• Maintain their professional registration.
• Participate in continuing professional development opportunities to ensure that up-to-date evidence based knowledge and competence in all aspects of the role is maintained.
• Work with the latest code of professional conduct (NMC etc).
• Record accurate consultation data in patients records in accordance with the latest professional guidance and other pertinent standards.
• Keep up to date with pertinent health related policy and work with the practice team to consider the impact and strategies for implementation.
• Work collaboratively with colleagues within and external to the practice.
• Demonstrate leadership.
• Pro-actively promote the role of the ACP/ANP within the practice and externally to key stakeholders and agencies.
• Participate in multi-disciplinary protocol and patient group directions development.
• Identify appropriate opportunities to delegate both clinical and administrative tasks to more junior staff.
• Help the practice operate in a cost effective manner.
• Identify and manage nursing care risks on a continuing basis.
• Participate in practice meetings, audits and inspections as appropriate.
• Work closely with the doctors and administrative managers in the setting up and/or improving of practice systems for monitoring/ measuring performance against Clinical Governance and quality indicator targets.
• Ensure that all practice policies are fully implemented.

Personal/ Professional Development:

The post-holder will participate in any training programme implemented by the practice as part of the employment, such training to include:

• Participation in an annual appraisal, including taking responsibility for maintaining a record of own personal and/or professional development.
• Attending relevant professional courses and study days.
• Ensure they are carrying out regular clinical audits of prescribing, consultations and referrals.

Communication:

The post- holder should recognise the importance of effective communication within the team and will strive to:

• Communicate effectively with other team members, including the Community team, Social Services and staff from Voluntary Organisations as required.
• Communicate effectively with patients and carers.
• Recognise people's needs for alternative methods of communication and respond accordingly.

The post- holder will assist in promoting and maintaining their own and others health, safety and security as defined in the Practice Health and Safety Policy in line with the Health and Safety at Work act (1974) to include (but not limited to):

• Using personal security systems within the workplace according to practice guidelines.
• Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks.
• Making effective use of training to update knowledge and skills.
• Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards.
• Awareness of national standards of infection control, cleanliness, and regulatory/ contractual/ professional requirements and good practice.
• Reporting potential risks as identified.
• Responsible for the correct and safe management of specimens process where appropriate
• Safe use of sharps.

Knowledge and Skills

Essential

• Good computer skills.
• Excellent written and verbal communication skills.
• Experience using clinical systems.

• A working understanding of NHS policies, processes, structures, systems, and clinical practices.

Qualifications

Essential

• A relevant Master's Degree (such as advanced clinical practice) with ability to demonstrate working at a level that encompasses the four pillars of clinical practice.
