Job summary

Consett Medical Centre is looking to recruit a Pharmacy Technician to join an established pharmacy team to maximise safe, cost-effective prescribing to improve the quality of patient care.

Main duties of the job

The postholder will help patients get the best from their medicines, use existing protocols to maximise cost-effective prescribing, improve repeat prescribing processes, minimising clinical risk and reducing avoidable wasted medicines

In addition, the post holder will develop and encourage a positive culture of medicines optimisation principles to assist the practice achieve local and national medicines related objectives.

Job description

Job responsibilities

General Responsibilities

  • Support the practice team to ensure efficient medicine optimisation processes are being followed.
  • Follow shared care protocols and increase the quality and safety of prescribing through medicine safety initiative and project work.
  • Respond to medication related queries generated by patients, GPs, other health care professionals and staff including repeat prescribing, medication review, medicines reconciliation and supply issues with medications.
  • Review of and actioning of clinic letters and hospital discharge summaries for changes to medication and medication related monitoring.
  • Acting upon tasks arising from secondary care correspondence.
  • Support of the practice reception team in streaming general prescription requests, to enable GPs, nurses and clinical pharmacists to review the more clinically complex requests.
  • Support patients to manage their medications, including provision of patient information and medication counselling, arrangement of monitored dosage systems, synchronisation of patient's medication and setting up of electronic repeat dispensing under the supervision of the pharmacy team.
  • Work with practice team to highlight patients who require ongoing monitoring and to promote patients to attend appointments for long term monitoring.
  • Action acute medication requests.
  • Maintain accurate and contemporaneous record keeping in clinical notes.
  • Support the practice to deliver PCN DES, QOF, QASI and locally commissioned enhanced services.
  • Support the practice in reviewing and developing practice policies for CQC requirements.
  • Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings.
  • Present at patient group meetings or appropriate events to give advice on the appropriate use of medicines.
  • Attend and participate in training programmes as a part of continuous education.
  • Ensure adherence to own professional revalidation and ensuring own competency is up to date for current role.
  • Assist with the provision of medication related training and education sessions for the practice team and other clinical and non-clinical teams.

Excellent interpersonal skills including ability to work as part of a team and the ability to motivate others.

Effective communication skills.

Ability to organise and prioritise workloads.

Demonstrate tact, diplomacy and tolerance at all times, with utmost importance given to patient care.

Experience of prescribing within clinical systems.

Reports to the Practice Pharmacist Team on a day-to-day basis.

Liaise with all practice staff, attached staff, external agencies including CCG staff and members of the public.
