Job summary

We are looking to recruit a Practice Nurse with previous experience working in Primary Care for 24 hours per week, over three days. We are looking for an enthusiastic, proactive Practice Nurse to work as part of our established and supportive team. The candidate will provide a high standard of care for patients and will work closely to support the practice partners and other members of the healthcare team.

Main duties of the job

To develop, implement and embed health promotion and well being programmes. To be qualified to deliver support to patients with long term conditions such as Asthma, COPD and Diabetes alongside general practice nurse duties including childhood immunisations, adult vaccinations, screening programmes and wound care.

Beacon View Medical Centre is a very friendly, busy, GMS practice with a list size of approximately 5,600 patients. We consist of 2 partners, 1 salaried GP, 1 Physician Associate, 2 Practice Nurses, 1 HCA, 1 Care Coordinator and an admin team of 6. We are a training practice so have doctors in training within the surgery. We are a supportive team with a good sense of humour and committed to giving the best possible care for our patients. We are part of the Gateshead Central South PCN.

Job responsibilities

JOB SUMMARY

To work with the Primary Health Care Team to identify and assess the core needs of patients of the practice including treatment, screening and advice within own professional competence

To maintain standards of care through evaluating and implementing evidence based practice

To provide nursing assistance to the doctors and other members of the team

SCOPE

To assist in maintaining standards of patient care

To support the PHCT in identifying needs through health needs assessment of practice/locality population

To participate in audit/practice development and staff development

Awareness of Government policy, national targets

CLINICAL DUTIES

Chronic Disease Management

To identify and manage as appropriate, treatment plans for patients with known Long Term Chronic Conditions including:

  • Asthma
  • COPD
  • Diabetes
  • Cardio vascular disease

Wound management

Perform wound care, suture and clip removal. Provide diagnosis, management plan and treatment of leg ulcers using Doppler assessment

Vaccination and immunisation

  • Provide an adult and childhood immunisation service
  • Develop practice protocols
  • Develop and work to patient group directives

Travel Health Advice

  • Offer a holistic approach to travel health including comprehensive advice to patients prior to travel including
  • Vaccination and medications
  • Safe sex
  • Food hygiene
  • Sun protection
  • First aid and emergency medication

Home Visits

On occasion there may be a requirement to undertake home visits for our elderly/housebound patients to carry out LTC reviews.

Other duties

  • New patient health checks/ NHS health checks
  • Elderly screening
  • Male Health
  • Women Health including cervical smears and family planning
  • ECGs
  • 24 Hr BP
  • Ear irrigation and advice on ear care
  • Venepuncture
  • Advice on general health care and minor ailments with referral to GP as necessary
  • Assist with any appropriate research projects
  • Act as chaperone when required by GP or other nurse

TREATMENT ROOM

  • Assisting in minor operations performed by doctors
  • Setting up practice protocols
  • Management of medical stores, dressings, drugs, medical instruments and diagnostic agents etc
  • Arranging cleaning and sterilising medical equipment

INFECTION CONTROL

  • Maintenance of medical equipment
  • Adopt appropriate infection control procedures as detailed in the Infection Control Policy and carry out regular audits.

ADMINISTRATION

  • To manage resources effectively
  • Updating practice computer when appropriate
  • Maintain accurate patients records
  • Carrying out computer searches and audits as necessary
  • Ensure safe storage, rotation and disposal of vaccines and drugs is undertaken
  • Awareness of Government policy, national targets etc
  • Attend and participate in any practice meetings when required

MANAGER OF RESOURCES
  • Develop and maintain a team culture
  • Contribute to practice business plan and use of health needs to ensure effective service planning
  • Support the effective implementation, utilisation and development of information technology systems within the practice to ensure accurate clinical and statistical data

LEARNING AND DEVELOPMENT

Maintain continued education by attendance of courses and study days as deemed appropriate for professional development ensuring revalidation requirements are maintained

Participate in education and training of staff and patients where appropriate

Participate in hosting pre-registration nursing students at the practice

  • Undertake mandatory and statutory training

Attend performance reviews and annual appraisal

FLEXIBILITY

The post holder may be required to work additional hours to cover sickness and holidays

Training and study days may need at times to be undertaken during your own time
