Main duties of the job

Your day will consist of:

  • The delivery of highly effective medical care to our patient population consistent with the GMS contract via face to face, telephone consultations and home visits.
  • Generic prescribing adhering to guidance.
  • Processing of administration promptly to include: referrals, correspondence, repeat prescriptions and test results.
  • As a total triage Practice, you will on a rotational basis triage alongside another GP allocating patients to appropriate practitioners in an appropriate timescale, whilst also dealing with some urgent patient appointments

About us

A forward thinking SystmOne (with eConsult & Ardens), Training Practice with a list size of almost 20,000 patients, operating across four sites in rural Northumberland located in Alnwick, Embleton & Seahouses. With an excellent diverse clinical team providing a good mix of male and female Partners, Salaried GP's, Physicians Associate, ANP's (Prescribing), Frailty Nurses, Practice Nurses, Nurse Associates and HCA's. We have named clinical & management leads and a number of clinical specialisms.

As a dispensing practice we also have a large pharmacy team consisting of: Prescribing and Dispensing Manager, Pharmacists, Pharmacy Technicians, Pharmacy Admin and Dispensers. Our PCN ARRS roles include: FCP's, Pharmacists, Pharmacy Technicians, GP Link Workers, Health & Wellbeing Coaches, Mental Health workers, Care Coordinators and a Clinical Research Team.

We hold daily MDT's with social care attendance, clinical education sessions and our team have access to Blue Stream training software

Job description

Job responsibilities

  • Provide high quality patient care including prevention, screening, treatment, management of chronic conditions and health promotion

  • Develop care and treatment plans in consultation with patients and in line with current practice protocols as well as wider guidance e.g. NICE

  • Carry out surgery, telephone and undertake home visits

  • Speak and see patients promptly and in a timely manner, in order to minimise waiting times.

  • Ensure appropriate follow up of patients

  • Generate, check and sign acute and repeat prescriptions in-line with practice policy

  • Liaise with other team members and organisations involved in patient care and respond promptly to queries

  • Investigate and refer patients as appropriate in line with current guidance

  • Deal promptly with results and correspondence, clearly communicating any actions that are required

  • Keep clear and and up to date computerised consultation notes and patient records to agreed standards and in-line with practice policies

  • Prescribe generically whenever appropriate and in accordance with the practice formulary, commissioning and national guidance

  • Maintain high levels of communication with patients and carers to ensure effective delivery of care

  • Demonstrate a commitment and contribution to clinical governance processes including significant event review

  • Demonstrate a commitment to life-long learning, audit and evidence-based best practice

  • Contribute to evaluation and audit within the practice

  • Assist with the training of other staff members and also with the education of doctors and nurses in training as appropriate

  • Develop and promote positive collaborative working relationships with partners and all team members

  • Liaise with all relevant agencies and services to promote knowledge and awareness of services in the practice and local community

  • Complete patient reports within a timely manner

  • Take responsibility for own personal development and participate fully in the GP appraisal scheme
