Rapid Response Physiotherapist North West Surrey Adult Services CSH Surrey Band 6 37.5hrs , 7 day service. We are looking for enthusiastic and motivated PT's to join our highly regarded Rapid Response Service and can offer either full or part time hours.
Based on the St Peters Acute Hospital site just of Jct 11 of the M25, we provide responsive community services to patients across the beautiful Thames and surrounding regions.

Working in our Multi-Professional team you will provide advanced levels of therapy practice to assess, investigate, diagnose, treat and prescribe rehabilitation packages of care to support patients in their own environment and preventing unnecessary acute hospital admissions.
Although expected to work autonomously you will not be alone, the team is supported by a clinical service manager and you will work alongside A&E consultants, GP’s Occupational Therapists, Advanced Nurse Practitioners, Community nursing and Social Care practitioners.
In this challenging role you will provide a holistic approach by:
• Assessing, stabilising, treating and managing patients in their own environment
• Independently and autonomously assess, diagnose, plan, implement and evaluate treatment / interventions and care for patients with attention to physical, emotional, social , cultural and mental health issues.
• Providing support such as advice, guidance and training on professional therapy issues, and taking part in clinical governance , using reflective practice, research, audit and clinical supervision
• Work in a collaborative and integrated manner with other stakeholders to ensure a seamless service delivery

• Excellent communication and time management skills
• Ability to work both independently and also as part of the wider multidisciplinary team
• HCPC Registration
• Demonstrate ongoing professional development and a commitment to providing high quality, safe and effective care that meets the needs of patients
• Ability to relate to patients and colleagues in a sensitive manner
• Flexible approach to work with the ability to work across a seven day service.
• Open to new ideas and training
• Have access and use of a car with ability to work across different locations

Successful applicants, who have not worked for CSH within the last 12 months, will receive an amount of £2000 paid in 2 instalments - £1000 upon commencement and a further £1000 following successful completion of probation.
