As part of this we have recently opened a Frailty unit and are looking for a Nurse Specialist for older people who shares our ambition and vision to deliver the best care for our patients.

We are looking for an experienced nurse with a specialist interest in Older people to work within a dedicated team consisting of a consultant geriatrician and therapy teams. Your role, as a specialist nurse will be to support the team with the clinical pathway and focus on reducing length of stay and readmission's in the best interests of patients.

You will need to have excellent general nursing skills, evidence of a masters or equivalent experience and evidence of specialist studies in elderly care. You should be confident working within a team and alone as well as prioritise your workload. Excellent communication skills are essential to act as an advocate for patients and to support carers/relatives. Completion of the Comprehensive Geriatric Assessment on admission, discharge planning, and liaison with community services will be a key features of this role.

You will be expected to attend the Trust’s Induction Programme for RNs, RMs and ODPs (which starts on the first day of employment). The programme is full-time (please contact the programme lead if this is problematic: nicola.henthorn@nhs.net), lasts for two weeks and will involve travelling to both Trust sites. The programme includes supernumerary shifts in the clinical area and classroom-based study days which run from 9am to 5pm.
