Consultant Care of Older People - Geriatric Medicine

We are entering an exciting period of development within Elderly Medicine at Warrington & Halton Hospital Foundation Trust. To support care pathway development and with new investment we have two Job opportunities for Consultant Physicians in the ‘Care of Older People Directorate’. These posts will form the basis of an integrated service offer with opportunities to work in and lead the development of older people’s service both in hospital and community settings. We are keen to recruit both newly accredited and experienced Consultants who will be fully supported to work with us to develop our strategy and deliver high quality services for older people. The posts will be for 8 programmed activities with 2 PA allocated for supporting activities. The option to work less than full time will be fully supported. We are looking for enthusiastic, forward thinking clinicians who can demonstrate excellent communication skills, enjoy working within a team and have the ability to innovate and lead. The successful candidates will be joining a fantastic team committed to providing specialist geriatric expertise throughout the patient journey. We are also looking to develop new areas of working so candidates with a sub-speciality interest will be accommodated and supported as much as possible. There will be no on call commitment after 8pm, however, there is the opportunity to join the medical on call rota if desired.

The Directorate of Care of Older Persons is within the Division of Acute Medicine, which is run as distinct Clinical business units and Care of Older Persons sits within Specialist Medicine. Acute inpatient, enablement and outpatient services are currently based at Warrington Hospital whilst some interim care is offered at our Halton site. WHHFT are part of the National Frailty Network and are implementing an integrated frailty pathway working across the whole hospital, recognising that frailty is ‘everyone’s responsibility’. The frailty pathway will be embedded into processes and be key to enhancing both the patient and staff experience of WHHFT. We are looking at an evolving process with a 3 year target for full implementation.

The Clinical Directorate of Elderly Medicine is currently supported by 10.5 wte consultants and their junior teams. We have Medical Speciality Consultants who manage respiratory, cardiology and gastroenterology patients.

Complex elderly clinics are provided on-site with consultants specialising in falls and supported by specialist nurses as well as general elderly medical clinics. We also currently offer acute access clinics three times a week and are planning on expanding this to 5 days per week with the addition of an ambulatory care area specifically for older people.
