Walsall Healthcare NHS Trust provides integrated acute and community Healthcare in the West Midlands serving a population of 260,000 residents. Walsall Manor Hospital houses the full range of district general hospital services. The £170 million development was completed in 2010 and we are continually upgrading. Construction of the new integrated critical care unit has commenced and is due to be completed 2017, and plans to extend the neonatal unit are underway.

We provide high quality, friendly and effective community health services from multiple sites covering Walsall. Our multidisciplinary services include rapid response and homebased care, so that those with long term conditions and the frail elderly can remain in their own homes.

An exciting opportunity has arisen for an experienced, enthusiastic, highly committed and self-motivated Senior Registered Nurse or Allied Health Professional to join our multidisciplinary specialist Community Stroke Service in the Cannock Chase Locality as Associate Team Leader.

We are seeking a dynamic and innovative individual who is able to demonstrate a sound knowledge of community rehabilitation, stroke care and have excellent communication and clinical leadership skills.

You will work closely with the Clinical Team Leader for Specialist Community Stroke Services to provide both clinical and management leadership to the team. You will take an active role in the continued development of the community stroke care pathways which includes both Early Supported Discharge and Community Stroke Rehabilitation.

The role also involves an individual caseload of assessing, treating and reviewing patients’ needs as required by the service. You will also be required to maintain links with stroke specialist teams in the acute sector and other community teams that supports integrated multidisciplinary approach to care delivery.

Professional development is actively encouraged and supported in a variety of ways. The service is committed to excellence in stroke care, with an emphasis on evidence based practice, patient involvement and professional development.

The service currently operates a 5 day Mon – Fri service between the hours of 830am – 4.30pm. The post holder will be required to contribute to 7 day working if the demand of the service requires it, therefore a flexible approach to working hours is required. You must be willing to travel throughout the working day across the Borough as the post requires.

Car driver/owner is essential for this post.
