In line with the Five Year Forward View and the Herefordshire and Worcestershire Sustainability and Transformation Partnership, South Worcestershire Alliance has a strong local focus and commitment to secure new, sustainable models of care via the establishment of integrated community teams, known as Local Neighbourhood Teams.

We believe that the introduction of Local Neighbourhood Teams provides the essential foundation upon which to build and develop out of hospital care and improve patient experience – with teams built around GP practices.

Our vision is to secure Local Neighbourhood Teams with ‘one team identity’ working to an agreed set of goals and priorities regardless of who employs different staff. This will improve coordination of care and communication and reduce duplication.

Join us at the start of our journey within Worcester City where we are currently setting up phase 1 of Integration. The vision is to have 2 Neighbourhood teams within Worcester City.

We have the following permanent posts available.

  • Community Nurse BAND 5 – part time and full time hours available on a permanent and fixed term basis.

    Community nursing experience is desirable but not essential. We are looking for a highly motivated, flexible team player with a broad range of nursing skills. Excellent written and verbal communication skills are also essential.

We aim to deliver an effective, high quality service that puts patients at the centre of their own care. You will work closely with GPs, Social Care, the Voluntary and private sector, Community Therapists, Advanced Nurse Practitioners, working across the planned, rapid response and proactive pathways.

You will be working with adults and older adults with a wide range of needs/conditions in their own homes and clinic settings to minimise unnecessary hospital admissions, reduce excess length of stay in hospital and support patients during end of life care.

Due to the nature of the posts the applicant must be able to travel independently / have access to a suitable vehicle for business purposes and hold a full UK driving license.

New employees to the Trust are required to pay the fee for the initial DBS check (this will be deducted from their first months’ salary) and the annual subscription (£13) to the DBS Update Service. The Trust offers a preceptorship year for newly qualified nurses and AHP's and attendance at preceptorship programmes is a condition of employment.

Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

Tier 2 Certificate of Sponsorship
