This is an exciting opportunity for an enthusiastic Pharmacist who is keen to work closely with patients in their own homes to join our expanding Medicines Advice at Home Team. The role will support the delivery of the Portsmouth Medicines Optimisation strategy for patients with unmet pharmaceutical needs in the community. This will involve targeted reviews for patients with medication related problems to minimise unnecessary hospital admissions, improve medicines adherence and to reduce inappropriate medicines use and waste. This is a full- time position. The post is based at St.Mary's Community Health Campus in Portsmouth. We welcome applications from Pharmacists currently working in either community or secondary care.
