An exciting opportunity has arisen for dynamic individuals to join the Community Learning Disability Team following a review of our service provision. The successful candidates will be based in a multidisciplinary team based in the West and South Community Learning Disability Team

The Community Learning Disability team provides multi professional health services for adults with learning disabilities who have a range of health needs which are unable to be met within mainstream services due to complexity and / or physical and sensory impairments.

The multi-disciplinary team are looking for a dynamic person to take on the role of a Band 6 Community Nurse. The ideal candidate will have knowledge skills and experience in providing a nursing service within community settings. You will have the skills to adapt quickly to the challenges of working within a multidisciplinary service

You must be able to demonstrate good caseload management and experience of working with adults with a learning disability and additional complex needs.

You will have had experience of supervising and mentoring junior nurses and support workers.

**An ability to travel throughout the Leeds area to meet the demands of the post is essential.**
