An exciting opportunity has arisen for an experienced LD nurse to join the Nottingham CityCare Partnership Continuing Care Team as a Continuing Care Nurse Co-ordinator. Nottingham CityCare Partnership is the leading provider of NHS community services in Nottingham; a staff-led social enterprise; trusted to connect with and care for people, to improve the health and wellbeing of the community.

Nottingham CityCare is looking to recruit a forward-thinking, resourceful, experienced and confident nurse to the post of LD Continuing Care Nurse Co-ordinator. The role of the nurse Coordinator will be to facilitate the implementation of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care within Nottingham City.

The role will involve providing advice, undertaking health needs assessments and coordinating continuing healthcare funding. You will carry out work autonomously and in partnership with health and social care professionals in arranging care packages and placements. You will be part of the Continuing Care Team which is based at Aspect House. The Co-ordinator will work in the Nottinghamshire community and also be required to cover the Nottingham hospitals.

The post is 37.5 hours per week.

Main duties of the job

The LD Nurse Coordinator will be responsible for carrying out Continuing Care assessments and reviews for NHS Funded Nursing Care Contributions, including any formal reviews that may need to be referred to the Continuing Care Panel.

They will also be responsible for the coordination of a Decision Support tool making sound and robust recommendations, contributing to social work assessments to identify the nursing needs of clients with complex needs or those applying for long term care.

You will work as part of a team but also expected to work autonomously, as a health professional, in a local authority or health setting, to provide professionally accountable decisions and reports regarding clients eligibility for NHS continuing healthcare. In addition, you will be expected to provide support across Nottinghamshire and neighbouring ICB's and local authorities in the provision of health care assessment as required.
